Patient request to amend medical records

Oakfield Health Centre aims to keep accurate, safe, and up-to-date medical records. If you believe information in your GP record is wrong, incomplete, misleading, duplicated, or recorded under the wrong patient, you can ask us to review it.

Examples include incorrect personal details, an incorrect diagnosis code, a medication or allergy error, or historical information that you believe is no longer accurate. Please tell us exactly which entry you are concerned about, why you believe it is inaccurate, and what correction or clarification you are requesting.

Medical records are legal and clinical records. We cannot usually delete historical information that was accurately recorded at the time, including letters received from hospitals, CAMHS, community services, or other healthcare providers. However, we can review whether a GP-coded entry should be corrected, recoded, clarified, or annotated.

Where you disagree with a historical clinical opinion, we may be able to add a note to your record stating that you dispute the entry. Where previous information was provisional, historical, or has since been superseded, we may be able to add clarification so that it is not used in future referrals as if it were a confirmed current diagnosis.

To request a review, please complete our medical record correction request form or write to us with the relevant details. We will normally respond within one calendar month. Complex requests may take longer, and we will let you know if more time is needed.
